How to Become a Hope for Cats, Inc. Volunteer:

1. Fill out the Application below completely and be sure to type in your full name as signature on the last page. Read, sign, and date both the Waiver, Release, & Indemnification Agreement and the Volunteer Agreement. Submit your application.

2. Our Volunteer Coordinator will contact you and schedule your orientation and tour of the cat shelter, Jake's Place.

3. After orientation you will be assigned to a weekly shift at Jake's Place. A trainer will help you learn the routines to make sure the cats and volunteers remain safe, healthy, and happy.

4. If you don't want to volunteer at the cat shelter, other opportunities include fundraising activities, cat transport to vet appointments and shows, and conducting adoption shows at local pet stores.

HFC WAIVER, RELEASE, AND INDEMNIFICATION AGREEMENT

This Agreement is entered into with Hope for Cats, Inc. jointly by the undersigned (type your name), in order to permit the Volunteer to participate in the volunteer program. This Agreement is for the benefit of Hope for Cats, Inc. and each of its officers, directors, agents, and representatives (known individually as an "Indemnitee" and collectively as "Indemnitees").

Volunteer is advised that the activity of working with the shelter cats can be hazardous and involves contact with cats that are unpredictable. As such, Hope for Cats, Inc. cannot be held liable for injuries or accidents that may occur as a result of working with the cats or for any other mishap either at the Jake's Place Cat Shelter and Adoption Center or when engaged in other volunteer activities for Hope for Cats, Inc.

Volunteer is aware that injuries, loss of or damage to personal property, and death may occur as a result of Volunteer's participation at the shelter or other Hope for Cats, Inc. - sponsored activities. Volunteer agrees that Hope for Cats, Inc. and Indemnitees shall not be held responsible or liable for any personal injury or other injury, including death, damage, loss, or expense to Volunteer or his/her property, whether or not such injury, death, damage, loss, or expense is caused by negligence of Hope for Cats, Inc., any Indemnitee, or a third party.

Volunteer and his/her heirs, executors, and administrators agree to hold harmless each Indemnitee against any and all manner of legal actions, such as suits, debts, claims, or liability of any kind incurred while the Volunteer participates at the shelter or in any other volunteer activity for Hope for Cats, Inc.

Volunteer fully, completely, and unconditionally waives and releases each Indemnitee from all rights, liabilities, duties, claims, charges, demands, actions, damages, costs, attorney fees, or expenses of any kind that Volunteer may have now or in the future against Hope for Cats, Inc. or any Indemnitee relating to participation at the shelter or in any other volunteer activity for Hope for Cats, Inc. or relating to any medical treatments authorized by any Indemnitee in accordance with the terms of his/her Volunteer Agreement.

Volunteer represents and warrants that he/she has the authority to enter into this Agreement.

If any provision of this Agreement is found to be unenforceable in any way, it shall be enforced to the maximum extent possible and all other provisions of this Agreement shall remain in full force and effect.

HFC VOLUNTEER AGREEMENT

In signing this agreement, I understand and agree to the following:

I will treat all animals, people, and property I come in contact with at any Hope for Cats, Inc. event and at the Jake's Place Cat Shelter and Adoption Center with respect. I will refrain from using profanity and conduct myself with courtesy at all times.

I understand that my services are provided in a volunteer capacity without any right to compensation or employment-type benefits and that I am not eligible for New York State Workers' Compensation or Disability Benefits in connection with my services.

I will reliably and consistently show up for my scheduled shift. If I cannot make it, I will contact all other volunteers until one agrees to cover for me. If I cannot find coverage, I will contact the Volunteer Coordinator, with a reasonable amount of notice. I understand that if I have not reported for my scheduled shift, and have not found replacement coverage, for three consecutive weeks, my active status as a volunteer will be terminated.

I will conscientiously complete all necessary paperwork during my shift, such as the chores checklist or health checklist, as related to my specific role.

I will immediately notify the Volunteer Coordinator of any accident, injury, theft or other unusual incident that occurs during my shift.

I understand that as a volunteer I may gain access to information about Hope for Cats, Inc., Jake's Place Cat Shelter and Adoption Center, Board members, other volunteers, or adoption clients that is confidential. I agree to maintain confidentiality and to refuse disclosure of any information that is either private or personal.

I am physically and mentally fit to safely work with animals and the public at the shelter or at any other volunteer activity for Hope for Cats, Inc. If an accident or other medical emergency occurs while I am volunteering at the shelter or en route to or from the shelter or any other event participated in or sponsored by Hope for Cats, Inc., and Hope for Cats, Inc. representatives are unable to timely reach my Emergency Contacts for medical authorizations, I hereby give my consent for Hope for Cats, Inc. representatives to authorize medical treatments.

When I am no longer able to continue as a volunteer with Hope for Cats, Inc., I will notify the Volunteer Coordinator and return the keys to the Jake's Place Cat Shelter and Adoption Center.

I give permission to Hope for Cats, Inc. to use photographs or video footage of my volunteer activities.

I give permission to Hope for Cats, Inc. to verify the information in my volunteer application.

Volunteering for Hope for Cats, Inc. and at the Jake's Place Cat Shelter and Adoption Center is at-will. Active volunteer status may be terminated for any reason by either party - the volunteer or Hope for Cats, Inc.
